FIRST APPOINTMENT

Your initial visit will include a consultation detailing your diagnosis and choice of management. Sometimes, surgery can be done on the same day as the consultation. Although, an elaborate medical history or treatment option deserves thorough evaluation and a second visit to schedule treatment for a set date.

Kindly provide the required information below at the time of your consultation to help us serve you better:

  • Your surgical referral form and any x-rays if applicable.
  • Current medication(s) you take.
  • If you have dental or medical insurance, come with the necessary, filled forms. This saves time and enables processing of any of your claims.

IMPORTANT: All patients below the age of 18 must be followed by a parent or caregiver at the consultation appointment.

All patients that are to undergo IV anesthesia for surgery must attend a preoperative consultation and do a physical examination. Please ensure 8 hours fasting before your surgery. An adult will also be needed to take you home.

Please let us know about any medical condition that may be of concern before your surgery (e.g., high blood pressure, artificial heart valves and joints, diabetes, rheumatic fever, etc.) or if you are on any routine drugs (i.e., aspirin, heart medications, anticoagulant therapy, etc.).

X-Rays

If your doctor or dentist has taken x-rays in the past, you may ask them to send to us in our workplace. At insufficient times, kindly collect them and bring to our office. If we require additional films, we can capture them in our organization.
